Osaamistavoitteet
The student can assess the health and functional capability of patients with non-communicable diseases, and their needs for care and rehabilitation. The student can make clinical decisions as an expert in nursing by utilizing decision-making supports, information systems and databases together with the patient and a multi-professional team. The student can apply evidence-based treatment methods and evaluate their effectiveness. The student is able to act as a clinical expert when planning and developing the prevention and treatment of non-communicable diseases.

The Chronic Care Model in the prevention and treatment of chronic diseases.

Arviointikriteerit, tyydyttävä (1)
The general criteria of the competences that the Master's Degrees awarded by Finnish Universities of Applied Sciences provide can be found on JAMK's web pages (http://opinto-oppaat.jamk.fi/en/Study-Guide-Masters-Degrees/). The learning outcomes of individual courses are assessed in relation to the objectives of the course concerned. Assessment is based on knowledge, skills and competence in accordance with the National and European Qualifications Framework (FiQF/EQF), level 7.
1 (Adequate)
The student can treat patients with essential non-communicable diseases in urgent situations, and carry the responsibility for the follow-up and assessment of the treatment and the guidance of self-care in chronic cases, in accordance with care recommendations and principles. The student can assess the practices of treating non-communicable diseases and, within the framework of the health benefit model, develop them towards an evidence-based approach utilising some information sources.
2 (Satisfactory)
The student can treat patients with essential non-communicable in urgent situations, and carry the responsibility for the follow-up and assessment of the treatment and the guidance of self-care in chronic cases, in accordance with care recommendations and principles. The student can assess the practices of treating national diseases and, within the framework of the health benefit model, develop them towards an evidence-based approach utilising a variety of information sources.
3 (Good)
The student can treat patients with essential non-communicable diseases in urgent situations, and carry the responsibility for the follow-up and assessment of the treatment and the guidance of effective self-care in chronic cases, in accordance with care recommendations and principles. The student can critically assess the practices of the treatment of non-communicable diseases and develop them towards an evidence-based approach by utilising the health benefit model, various information sources and multidisciplinary evidence from a variety of perspectives.
4 (Very good)
The student has mastered the treatment of patients with essential non-communicable diseases in urgent situations, as well as the follow-up and assessment of the treatment and self-care guidance in chronic cases, in accordance with care recommendations and principles. The student can critically assess and compare the practices of the treatment of non-communicable diseases and develop them towards an evidence-based approach utilising the health benefit model, various information sources and multidisciplinary evidence in a diverse manner.
5 (Excellent)
The student has mastered the treatment of patients with essential non-communicable diseases in urgent situations, as well as the follow-up and assessment of the treatment and self-care guidance in chronic cases, in accordance with care recommendations and principles. The student can critically assess and compare the practices of the treatment of non-communicable diseases and develop them towards an evidence-based approach utilising the health benefit model, various information sources and multidisciplinary evidence in a diverse manner and with justifications.
If the student does not meet the minimum criteria set for the course, the grade is 0 (Fail).
