Objective

The student:
- Is familiar with the significance of the rehabilitation process on different levels (individual, community and society).
- Knows how to plan, implement and assess different rehabilitation processes and services together with the client and the multidisciplinary work group.
- Knows hot to initiate multidisciplinary collaboration and function as an active member of a work group.
- Recognises the principles of coordinating multidisciplinary collaboration in the work environment in question.
- Knows how to compile and document the information produced collaboration and how to use it when implementing and assessing the rehabilitation plan.
- Knows how to choose an use effective rehabilitation methods and justify the choices.
- Recognises their role as a part of the work community and the development of practices.
- Knows how to reflect on their actions as a member of the interdisciplinary work group.

Content

Analysing the rehabilitation process from the perspective of the individual, the communit and the society.
Planning, implementing and assessing the rehabilitation processes of different clients as a member of a multidisciplinary work group.
Learning about coordinating multidisciplinary collaboration.
Compiling, documenting and assessing rehabilitation plans.
Learning about various rehabilitation methods and how to use them based on theoretical information.
Development work.
Reflecting on, sharing and documenting one's own skills.

Completion alternatives

The student - Is familiar with documentation and uses it to implement the rehabilitation plan. - Plans rehabilitation processes and services together with the client and the multidisciplinary work group by applying information related to social and health services and current legislation in various ways. - Is able to negotiate with different actors in the rehabilitation network and supports its operations with their expertise. - Uses effective rehabilitation methods and justifies them using theoretical information. - Recognises and presents the areas of development within the work community in a constructive manner. - Recognises their own role in the multidisciplinary work group.

Opiskelija
- Hallitsee dokumentoinnin ja hyödyntää niitä kuntoutussuunnitelman toteutuksessa.
- Suunnittelee kuntoutusprosesseja ja -palveluita yhteistyössä kuntoutujan kanssa ja monialaisen työryhmän jäsenenä soveltaen monipuolisesti sosiaali- ja terveyspalvelujärjestelmää sekä voimassa olevaa lainsäädäntöä koskevaa tietoa.
- Toimii neuvotellen kuntoutusverkostossa ja edistää asiantuntijuudellaan sen toimintaa.
- Käyttää perustellen teoriatiedolla vaikuttavia kuntoutuksen menetelmiä.
- Tunnistaa ja tuo rakentavasti esiin työyhteisön kehittämiskohteita.
- Tiedostaa oman roolinsa osana monialaista työryhmää.

Qualifications

The student knows the rehabilitation service system and on related central legislation. The student knows the basics of the rehabilitation, operative forms, actors and customer groups of rehabilitation. The student knows the basics of effectiveness and planning of the rehabilitation.
